What to expect:

  • A new, comprehensive ocular examination plus dilation will take about 45-60 minutes - add additional time if you wear contact lenses and even longer if you wear specialized contact lenses.
  • New patients should arrive no less than 15 minutes prior to your visit (30 if you need to finish paperwork). 
  • All new patients should plan to be dilated at the exam, so if you are uncomfortable driving after being dilated, please arrange for a driver.
  • Once you arrive feel free to have some complimentary coffee or tea.

What to bring:

  1. **New patients: print and complete all paperwork below prior to your visit.
  2. Bring your medical and vision insurance cards if you plan on using your benefits during your visit.
  3. Bring all glasses, contact lenses, and list of current medications.
  4. Full name, date of birth, last 4 digits of SSN of patient (and insurance subscribers' info, if different.)
  5. If insurance subscriber is different than patient, provide info listed above.
